  • Short-Term Disability & Long-Term Disability are demographic based rates with calculations made in ADP

  • Voluntary Life– Employee, Spouse, and Child(ren) rates are demographic based and rate table calculations are made in ADP.

  • MetLife Voluntary Critical Illness are demographic based rates with calculations made in ADP.

  • MetLife Pet Insurance – Employees must call into MetLife to start a policy. Enrollments are not made in ADP. Rates are reliant on animal demographics.
